If you wish to access online services please download and complete the form and return it to the surgery with address and photo ID. If you have already downloaded the NHS App and confirmed with Citizen Identity you will not be required to provide further ID.

Should you wish to request proxy access to enable you to manage repeat prescriptions for another person please download and complete the form and return t to the surgery. If you require proxy access for a child please complete the form but it will not require a signature unless the child is over 13 years of age.
